What this site collects

When you send a message through the form, we receive your name, phone, email, how you would like us to reach you and your message. We use them to reply to you and to plan your visit. We do not sell them, and we do not share them with advertisers.

What to leave out of a message

Please don't include medical details in a message. Your doctor asks about your health in person, at your visit. For anything urgent, call us.

Your records at the office

Records from your visits are kept at the office. Ask the front desk if you would like a copy, or if you would like us to send them to another provider.
